Medication Management is the process of monitoring medications prescribed to patients, ensuring that the medicines are being taken appropriately and are meeting the intended therapeutic goals. When integrated into a healthcare team via Medication Management, the pharmacist works collaboratively with a patient, clinician, and other providers to establish and meet the best possible drug treatment goals.

It is most effective when patients receive medication management in the same practice where they are also receiving therapy because it is easy for providers to collaborate in their care and treatment plans. Essentially, medication-management therapy is essential for clinicians and mental health clinicians to make sure mental health patients are taking the best medications for their treatments and using them safely and effectively.

The Ministry of Transportation (MTO) is the provincial ministry in the Ontario government responsible for transportation infrastructure and associated legislation within Ontario.
