We commonly extrapolate findings from adult clinical trials to similar children populations as paediatricians, knowing that this is an inadequate method while lamenting the absence of well powered pediatric clinical trials. Clinical trials in children are challenging to complete for a variety of reasons. For example, certain illnesses may be less common and have different consequences in children than in adults. For example, mortality is widely used as an end goal in adult studies of intensive care unit patients, but mortality in the pediatric intensive care unit is low, making it an insensitive metric to use in determining the success of many therapies. Furthermore, there are unique considerations in moving forward with studies in the vulnerable demographic of children, such as a more complicated informed consent process and the potential for parental resistance as surrogate decision makers.
