Why was ivermectin discovered?
Ivermectin was discovered by a parasitologist in Nigeria. He noticed that people who worked in fields and were exposed to earthworms had fewer cases of river blindness than people who were not exposed to worms.

How does Ivermectin kill microfilariae?
Ivermectin tablets are an anti-filarial drug developed in 1987 by Merck. It kills microfilariae (larvae) of Dirofilaria immitis, a parasitic worm that causes heartworm disease, and Onchocercas volvulus, a parasitic worm that causes river blindness.

To eliminate them, experts say they must be dealt with at their source: in areas where they are most prevalent. That means treating entire communities rather than individuals.
WHO estimates that if ivermectin were distributed widely enough, it could reduce transmission rates by 99 percent within five years. If we want to get rid of [river blindness], we have to go after all three [mosquito-borne] diseases, said Dr. Peter Hotez, dean of Baylor College of Medicine's National School of Tropical Medicine and head of Texas Children's Hospital Center for Vaccine Development in Houston.
We need to go after dengue fever and chikungunya as well, he added. These three diseases are extremely important.
What are the next steps?
Currently, over 200 million people around the world are at risk of contracting river blindness (onchocerciasis).
Ivermectin is a new drug that could dramatically reduce rates of infection and disease. It’s given to cattle in Africa to prevent them from spreading onchocerciasis to humans; however, it’s not regulated by any international health authority.
Before ivermectin can be used as a potential treatment for river blindness in humans, an additional clinical trial will have to take place and also secure regulatory approval from international health authorities. Right now there’s no timeline for when these next steps might happen—or even who will pay for them.
What are the next steps in Africa?
The Carter Center and National Geographic recently brought together experts from around the world to discuss how we could combat river blindness in humans.
Experts agreed that ivermectin is a viable treatment, though doses will need to be adjusted based on a person’s weight and age.
Currently, ivermectin is not approved by the Food and Drug Administration (FDA) for use in humans, so there are barriers that must be overcome before people with river blindness can take advantage of these new treatments.
However, encouraging results could eventually lead to FDA approval. In addition, using ivermectin for humans may also provide an opportunity to help control dengue fever and other neglected tropical diseases. More research is needed before we know if these drugs would work as well in people as they do in animals.
